Where can I get a "nuc sold" form

Sorry if this is in the wrong part of the forum, but I wasn't sure where to post it: I am sure I saw a nicely laid out form a while back which you could just fill in which had all the info required when selling a nuc (names, addresses, origin, treatments prior to sale, etc). I have my own, but if nothing else would like to make sure I have the right amount of info and am adhering to whatever the "current guidelines" should be.

Any ideas?

At least I can now print the pdf out and attach it to each form I fill in.
