You are missing something somewhere.

Feed colonies before and continue feeding after.
Make sure the grafting tool is very smooth, polish as necessary to remove any roughness.
Pick up the larvae from the convex side (Back of the "C").
Bring as much royal jelly with it as possible, depress the cell bottom if necessary.
Do NOT hit the cell sides with the larva as you raise it, feel free to cut down the cells - that helps.
Do Not allow the larva to turn over.
Collect a few natural queen cups and attach to a cell bar, graft into those and see what happens.
Practise grafting with older larva to master the technique.
How are you preparing the cell cups? Could there be a problem there?
