Or it's negative? - your normal is pretty good success and this was the exception of a relatively poorer result.
I'm never sure if the expression really arose when a flintlock only made the pan ignite and flash but no full gunshot, so it would then be "an occasional failure" - exactly as you say.
Most dictionaries have it as more the "unrepeatable success"...

Saw an old Michael Palin travelogue recently where some tribesman went to discharge a venerable 6ft musket, and it only produced a small pan flash so the tribesman poured on loads of powder and had another go, which produced a huge bang from pan and muzzle and left Palin visibly shocked (and worried) by the impact from the mega flash-in-the-pan which tousled his hair for him and sent him staggering back a pace or two.
